After years of rising medical costs and sharp insurance premium increases, the Malaysian government is preparing to introduce a new basic medical and health insurance or takaful plan (MHIT) aimed at making private healthcare more accessible and sustainable.

The new MHIT plan will offer monthly premiums as low as RM80 to RM120 for younger adults, with a pilot programme scheduled for the second half of 2026 and a full rollout expected in early 2027.

The initiative is part of a broader national effort to curb rising private healthcare costs, improve price transparency, and ensure that Malaysians are not priced out of essential medical protection as they age.

Why the Government Is Introducing a Base MHIT Plan

Finance Minister II Datuk Seri Amir Hamzah Azizan said the base MHIT plan is designed to provide essential financial protection while improving how private healthcare spending is structured and managed.

He explained that the plan focuses on covering high-impact medical needs rather than luxury or excessive care, ensuring premiums remain affordable for a wider group of Malaysians.

“It will also enable more Malaysians to obtain basic-level financial protection to cope with essential and high-impact healthcare expenses,” he said.

The MHIT plan forms part of several initiatives proposed by a ministerial-level committee tasked with addressing the rising cost of private healthcare. The committee is jointly led by the Finance Ministry and the Health Ministry and involves Bank Negara Malaysia, academics and private sector stakeholders.

What the Base MHIT Plan Actually Covers

According to the White Paper on the Base MHIT Plan, the standard package offers annual coverage of up to RM100,000.

This coverage limit is based on recent medical claims data and trends, which show that it would be sufficient to cover 99 percent of treatment cases for common medical conditions, even when multiple hospital admissions occur within a year.

The base MHIT plan provides coverage for a wide range of private hospital services. These include:

  • Hospital room and board
  • Surgical and anaesthetist fees
  • In-hospital physician visits
  • Treatment-related medications
  • Selected high-cost outpatient drugs for serious illnesses such as cancer
  • Ambulance services
  • Intensive care unit and operating theatre charges
  • Day surgeries
  • Immediate pre- and post-hospitalisation care, including consultations, diagnostic tests and medicines

The intention is to ensure that patients are protected from major medical bills while keeping the plan focused on essential and necessary care.

How Much Malaysians Can Expect to Pay

Indicative monthly premiums under the standard base MHIT plan vary by age group.

For individuals aged 31 to 35, premiums are expected to range between RM80 and RM120.
For those aged 61 to 65, premiums are estimated at RM280 to RM350 per month.
For individuals aged above 75, premiums may range from RM500 to RM780.

A standard-plus MHIT plan will also be offered. This version comes with higher deductibles and coverage limits and is designed for individuals who require lighter or more occasional coverage, such as those already covered by employer benefits. Premiums for the standard-plus plan are expected to be lower than the base plan, although final rates will be announced closer to the 2027 launch.

Coverage Until Age 85 

One of the most significant features of the new MHIT plan is its focus on age inclusivity.

Coverage under the plan will extend up to the age of 85, while enrolment will be capped at 70 years old or younger, regardless of income level.

The base MHIT plan will be offered as a stand-alone medical protection product and will not be linked to investment or savings components. This is intended to ensure clarity, transparency and affordability.

Amir Hamzah also highlighted a key consumer protection feature for existing policyholders.

“Crucially, policyholders facing repricing will have the option to switch seamlessly to this base plan with their current insurer without new medical underwriting,” he said.

This allows individuals affected by rising premiums to move to a more affordable option without being rejected due to age or medical history.

Improving Price Transparency in Private Healthcare

Health Minister Datuk Seri Dr Dzulkefly Ahmad said the MHIT initiative goes beyond insurance pricing and also focuses on helping patients better understand healthcare costs before seeking treatment.

As part of the reforms, the insurance and takaful industry has published benchmark prices for 26 common medical procedures at private hospitals. These benchmarks show price ranges based on location and age group, as well as average lengths of hospital stay.

“This helps you compare prices and understand potential expenses before seeking treatment,” Dzulkefly said.

In addition, new digital tools will be introduced, including a health insurance readiness calculator, developed with support from Bank Negara Malaysia and the Employees Provident Fund. These tools are intended to help Malaysians plan their monthly premiums and potential out-of-pocket costs over time.

Why This Reform Matters Now

The MHIT reforms gained urgency following public backlash in late 2024, when many Malaysians faced sudden medical insurance premium increases of 40 percent to 70 percent.

Medical inflation remains a key challenge. It is projected to rise to 16 percent in 2026, driven by increasing healthcare operating costs. Without intervention, private medical coverage risks becoming unaffordable, particularly for older Malaysians.

The base MHIT plan is intended to serve as a baseline option within the market. It will co-exist with existing medical insurance products while encouraging better cost discipline, more consistent care standards and improved alignment between insurers, hospitals and patient outcomes.

For many Malaysians, the new MHIT plan could represent a more realistic and sustainable way to maintain private healthcare protection in the years ahead.
